The PAA can either be used in combination with the Prism as an evaluation and triage tool, or it can be used as a standalone tool to enable and facilitate Risk Stratification and Population Health Management. The second tool is the Patient Activation Assessment (PAA). The patient or service user’s self determined score against each of the 8 facets of the Prism allows the healthcare professional to evaluate and quantify the impact of their interventions on the health and wellbeing of their patients. These cover the majority of the complex conditions and circumstances of patients typically referred to social prescribing or similar Personalised Care services. The Prism itself is an 8-sided Prism, with each facet of the Prism representing a carefully selected grouping of the Social Determinants of Health. The Health and Wellbeing Prism is two evaluation tools combined in a single integrated platform. In the same way that a glass Prism is used to separate out and view the different wavelengths of light, so the Health and Wellbeing Prism allows healthcare professionals to separate out the different issues and challenges facing their patients and service users, and address ‘What matters to Them’ in a holistic and structured way, consistent with the principles of Personalised Care.
